• A picture with no description. A screen reader read its file name. Write good alt text
• Pale gray text, 2.3 to 1. Ordinary text needs 4.5 to 1, or people with weaker eyesight can't read it. Color check
• A “click here” link. A link's words should say where it goes. Accessibility Check
• A form box with no label. Nobody can tell what goes in it. Accessibility Check
